About Us

About Us

Care Plus Nursing and Rehabilitation, Inc. (CPNR) began as a small group of dedicated nurses and therapists, gathering after long workdays to share dinner on Friday nights. From those humble beginnings, we’ve grown into a leading therapy provider, serving patients through numerous home health agencies across the Chicagoland area.

What started with visits to just 11 patients has now expanded to an average of 250 active patients weekly. Today, CPNR proudly supports a team of 75 professionals, including Physical Therapists (PT), Occupational Therapists (OT), and Physical Therapy Assistants (PTA).

As part of our ongoing growth, we’ve also welcomed foreign-trained therapists into our workforce, continuing to expand our diverse and talented team.

At Care Plus, our mission is simple: to help our patients restore their health to its fullest potential. We are here to support you every step of the way.
